What Is FollowMyHealth

FollowMyHealth (FMH) is a FREE online tool that keeps you connected to your health information 24/7.

With FollowMyHealth you can:

  • View your medical records online in a safe, secure environment
  • Talk privately with your provider via secure messaging
  • View test and lab results
  • Read medical notes from your doctor
  • Update your health information (allergies, medications, conditions, etc.)
  • Request Rx refills

  Signing Up: Starts With a Call

  A new patient who has already called their nearest SICHC location office and have been set up to be a new patient, will need to look for an email invitation from “Follow My Health”.  

• To Create an Account from an Invitation:

  1. Click on the registration link that is provided in your email, it comes from ”noreply@followmyhealth.com”
  2. Select the “Sign Up and Connect” option
  3. Next is the username and password screen.  By default, your email address will be in the username field.

  If you share an email, or you want to use a different username, you can edit that field.

  1. Next you will need to read and agree to the user terms.
  2. Enter the Invitation Code, which will be either the last 4 of your SS# or your Birth Year.
  3. Click “Agree” to release of information.

  You will then be ready to securely access & manage your personal health record online from any computer, smartphone, or tablet.
